Campus Mental Health Partnerships: Case management toolkit

This toolkit contains forms, protocols, frameworks, survey tools, education and awareness materials created for the Campus Mental Health Partnerships: Extending the Circle of Care project. This second round Mental Health Innovation Fund project was led by Georgian College, in partnership with Centennial College. The aim of the project was to design, implement and evaluate a postsecondary mental health case management service delivery model; define roles and responsibilities for counselors, other specialized college staff and community partners; build collaborative relationships among internal college staff and external community partners; establish a systematic triage and referral process; and create a professional development framework and education & awareness plan to support a multi-partner case management model.
