This document includes four resources/forms that were developed by the Counselling and Accessibility Services department of Fanshawe College (London, Ontario). This department created a task group to identify current suicide risk assessment processes, guidelines, best practices and forms . The task group contacted 107 postsecondary campuses, community agencies and hospitals to seek their input and best practices. Scholarly publications were also reviewed. These findings were compiled and used to inform the development of the following resources:
